Responsibilities
- Personal Care: Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and meal preparation.
- Health Monitoring: Observe and document clients’ health and behavior, promptly addressing any changes or concerns.
- Medication Management: Administer prescribed medications and ensure adherence to individual care plans.
- Companionship: Provide emotional support and build trusting relationships to enhance mental well-being.
- Social Engagement: Facilitate and support clients’ participation in social and community activities to promote inclusion and interaction.
- Documentation: Maintain accurate and confidential records in compliance with HIPAA regulations.
- Collaboration: Work with healthcare professionals, family members, and other team members to create individualized care plans.
- Training and Compliance: Attend regular training sessions to ensure adherence to safety protocols and best practices in client care.
Qualifications
- Previous experience in group homes, assisted living, memory care, or home care settings is preferred.
- Familiarity with patient observation techniques and home care practices is a plus.
- Certification in CPR and First Aid is highly desirable.
- Strong communication and interpersonal skills, with the ability to build rapport and trust.
- Keen observation skills to recognize changes in clients’ health or behavior.
